An inmate has stabbed three prison officers at high-security Frankland jail in County Durham.
Two of the victims, a man and woman, have undergone surgery for their injuries while a second male officer suffered less serious wounds.
The attack, involving a broken bottle, happened at 08:30 GMT on Saturday. None of those injured is critically ill.
In 2008, HM Inspectorate of Prisons raised concerns about violence at the Category A jail.
Colin Moses, chairman of the Prison Officers Association (POA), said: "This incident shows the level of violence that our officers face on a daily basis at Frankland and other prisons".
Mr Moses said there was a level of violence "deemed acceptable" within prisons, and has called for a zero-tolerance approach.
A spokesman for the Ministry of Justice said: "Three prison officers at HMP Frankland were injured on Saturday 13 March in an incident involving a prisoner.
"All three officers were taken to hospital. The incident is now subject to a police investigation.
"Violence in prisons is not tolerated in any form. Assaults on prison staff are taken very seriously and depending on the severity will be dealt with internally or referred to the police."
